This course follows the investigation of an accident, showing what not to do and then the correct method. It can be presented as a single session or broken out into three sessions; it has been presented both ways.

• Single Session (4 hrs)
• 3 Sessions (1 ½ hr each)

Course outline:

• The initial response; what must be done immediately.
• Collecting evidence and information. How this is done.
• Analysis of the information; determining basic causes.
• Developing remedial actions for the short term and for permanent prevention.
• Reporting findings.

The program is video based with participant worksheets for recording the results of the discussions.
